Please be aware that the following forms are required for participation in our events for adults with disabilities Carroll County, MD. Each participant only needs to fill them out once, unless there are any updates. This process ensures we have current emergency contacts and permission forms on file. MEDICAL AUTHORIZATION

In the event of an emergency, I authorize Taylor’s Place Community representatives to obtain necessary medical treatment for me (or my minor child) if I cannot be reached. I understand that I am financially responsible for any medical expenses incurred.

TERM OF AGREEMENT

This Agreement is valid for a five (5) year period from the date signed and applies to all Activities during that time period.

ASSUMPTION OF RISK

I understand that participation in Activities may involve inherent risks including, but not limited to, physical activity, travel, interaction with others, use of equipment, or outdoor conditions. I voluntarily assume all risks associated with participation.

RELEASE & WAIVER OF LIABILITY

In consideration for being permitted to participate in Activities, I release, waive, discharge, and hold harmless Taylor’s Place Community, its directors, officers, employees, volunteers, agents, and representatives from any and all claims, liabilities, damages, or causes of action arising out of participation in any Activity, except in cases of gross negligence or willful misconduct.
